Vitiligo is due to stoppage of melanin formation by the melanocytes.
It is asymptomatic and presents with depigmented macules anywhere on the body including mucous membranes of lips and genitalia.
There is a marked reduction or even absence of melanocytes and melanin in the epidermis.
There is lack of DOPA positive melanocytes in the basal layer of epidermis.
Some patients have only few lesions others have multiple.
The lesions in some of them remain static or progress very slowly. while in others the disease covers very fast and cover whole body in few months.
Cause: It is supposed to be transmitted by an autosomal dominant gene with irregular penetration and 10-30% of patients have been found some relative having Vitiligo.
Vitiligo has also seen on flank of some ladies who wear tight petticoats. In such cases it is formed by tight pressure.
Vitiligo lesions have been found to occur very frequently on knees, elbows,ankles and knuckles.
In some patients of vitiligo lesions spread very fast. such types are frequently associated with other diseases like hyper and hypothryroidism, perinicious anaemia, Addison’s disease, diabetes mellitus, malignant melanoma and halo naevus.
Inspite of all these in a large number of cases still remains unknown.
Treatment:
Vitiligo is an harmless disease except for its cosmetic implications.
repigmentation can be achieved in the patients who undergo appropriate treatment.
Response to the treatment is slow.
The lesions in the hairy area shows early and quick response.
Repigmentation also starts at the margins of the lesions.
There is no use of continuing any treatment which does not produce desired results.
